116 - Assessing Resident Understanding of Common Pediatric Rashes in Skin of Color

Background: Medical training, specifically dermatologic training, has lacked in exposure to skin of color (SOC) education/images. A recent study by Silverberg et al (2021) noted that although the ACGME requires training in understanding health disparities and reducing them, there is still a deficit in education regarding dermatology on SOC. Nijawahan et al (2008) surveyed chief residents and program directors on type of education in skin of color within their programs, with only 52.4% of chief residents and 65.9% of program directors reporting specific education in SOC. Textbooks for residents are also lacking in representation. Recently, there is a lack of SOC representation with COVID-associated skin findings.

Objective: The objective of this lecture was to educate the Pediatric residents on common Pediatric skin rashes on SOC. The purpose of the surveys were to understand baseline previous teaching on skin rashes on SOC in medical school and/or residency and to determine the efficacy of the lecture regarding comfort with skin rashes on SOC.

Design/Methods: Surveys were completed prior to and after the lecture. Surveys were randomized by a code created using favorite color, date of birth, residency year, and year graduated from undergrad. Surveys were handed out prior to the lecture given and residents were given 15 minutes to complete. The lecture was then given to the residents and then post-lecture surveys were handed out after and the residents were given approximately 15 minutes to complete. The lecture included common pediatric skin rashes on SOC patients and the dermatological differences (healing) of SOC patients.

Results: 15 residents completed pre and post-surveys. 80% reported "some" dermatology experience in medical school, with 96% reporting "not much" in residency. Only 27% reported "some" education on rashes of SOC, which increased to 100% after the lecture. 86% reported feeling "somewhat" comfortable with rashes on SOC, with none reporting feeling "very comfortable", with an increase to 93% feeling "somewhat" comfortable post-lecture. 93% reported exposure to rashes on SOC in the clinical setting. 93% felt the lecture was helpful (either "very" or "fairly").Conclusion(s): The lack of dermatological education on SOC hasn't previously been well studied in Pediatric residency. This study demonstrated lack of SOC exposure/education in training. Many residents reported a lack of confidence in diagnosing pediatric rashes on SOC; though clinical exposure was not limited. The importance of training includes improving patient care. Limitations include the size of study, recall bias, and generalizability.
