Racial and socioeconomic inequities in child health outcomes are driven by upstream, population-level social determinants of health, including structural racism and economic injustice, and downstream, individual-level social risks, such as food insecurity and housing instability. Many pediatric health systems have integrated social risk/social needs screening and referral programs into their clinical practice. However, screening for unmet social needs can have unintended consequences – generating mistrust and even perpetuating inequities – particularly if screening is not accompanied by evidence-based, family-centered social care interventions.
Drawing on both the recent literature and our team’s years of experience implementing social care interventions in the inpatient, outpatient, and emergency department settings, we will present an engaging, interactive discussion of current challenges and opportunities for health systems aiming to equitably address families’ unmet social needs. In particular, we will focus on how social care interventions can be thoughtfully designed and implemented to address disparities in real world practice.
In this hot topic session, we will (1) provide an overview of the 2021 US Preventive Services Task Force recommendations for social risk identification, (2) describe challenges and opportunities for innovation related to social risk assessment and social care interventions, and (3) provide a framework for equitable, family-centered integration of social care with medical care. These presentations will be followed by time for audience feedback and discussion. Attendees will leave with a framework for promoting health equity through social risk assessment and intervention at their home institutions and an understanding of key research questions in this field.
