Prior to COVID there was a trend toward regionalization of pediatric care and loss of pediatrics beds in smaller and rural hospitals. The circumstances of COVID that produced increased need for adult beds and a relative sparing of pediatric patients caused an increase number of closures of pediatric inpatient services, many of which have stayed close. While there is clear data to support regionalization of specialty care and some diagnoses for which procedures are vital to treatment in order to maintain sufficient volume to maintain skills, the benefits of transferring children with more common diagnoses are less clear. For all transferred children there are the risks and costs of travel, usually increased costs of care at tertiary care centers, and increased burdens on the patients' families. Coincidentally the response to COVID has also brought forward the use of telemedicine to provide service, more so in lieu of ambulatory visits but also for inpatient care. In the background of all of this is the first Pediatric Hospital Medicine board exam in 2019 created over 1400 certified pediatric hospitalists with the next test now scheduled for 2022 and there are now over 60 ACGME certified PHM fellowships. There are estimated to be about 3000 currently practicing pediatric hospitalists, a majority of which are in community settings. The panelists will discuss the challenges moving forward about the further consolidation of pediatric care and how that impacts access to care, care delivery, pediatric care costs, and the pediatric workforce.
