Background: Due to the limited treatment options, maternal psychiatric disorders during pregnancy continue to be a major challenge. There is a scarcity of information on neonatal comorbidities and long-term neurodevelopmental outcomes of infants born to mothers who have psychiatric disorders

Objective: To evaluate the effect of maternal psychiatric disorders such as Schizophrenia, bipolar disorder, major depression, generalized anxiety disorder) on neonatal morbidity and neurodevelopmental outcomes in a cohort of infants.

Design/Methods: Retrospective cohort study of all live births at Baylor Scott & White Health, from 2009 through 2014. The presence or absence of a maternal psychiatric diagnosis during pregnancy was used to group the infants. Demographic, clinical, and laboratory data were reviewed. Follow-up data were collected on developmental delays diagnosed at 2-7 years of age. The primary outcome was the diagnosis of developmental delay in childhood, defined by ICD-9 and ICD-10 codes. Secondary outcomes included childhood diagnoses of Attention Deficit Hyperactivity Disorder (ADHD) and autism spectrum disorder. A logistic regression model controlling for significant variables was used to generate odds ratio.

Results: 15,002 live births to 12,406 mothers were included in the study. 4168 (28%) of infants were admitted to the neonatal intensive care unit (NICU). 969 (6.5%) infants were born to mothers with a psychiatric diagnosis during pregnancy during the study period. In multivariate analysis, controlling for maternal age, race, and NICU admission (prematurity and other acute problems), maternal major depression during pregnancy was associated with conduct disorder in childhood (OR 1.8 [95% CI 1.28, 1.2.65]). Furthermore, the risk of developing autism spectrum disorder (OR 1.9 [95% CI 1.10, 3.18]) was significantly increased in infants born to mothers with major depression and bipolar disorder simultaneously.Conclusion(s): Children born to mothers with major depression are at an increased risk for being diagnosed with a developmental delays and those with additional diagnose of bipolar disorder have a two-fold risk of developing autism spectrum disorder, when controlling for other related factors. Further research is needed to determine the role of psychiatric medication consumption during pregnancy on developmental delays.
