Background: Disorders of gut-brain interaction (DGBI) are increasingly common and linked to stressors in adolescents. Residential moves have been identified as adolescent stressors; their impact on DGBIs is unknown.

Objective: To evaluate the relationship between moves and DGBIs in adolescents.

Design/Methods: This retrospective cohort study used Military Health System Data Repository data to examine the impact of recent moves on adolescent DGBIs at age 17-18. 325,663 children identified at age 10 were followed until 18; child sex, military parent sex, marital status, and rank were captured. Attention-deficit/hyperactivity disorder (ADHD), obesity, and mood disorders were identified by ICD-9/10 codes. Total moves were categorized as 0, 1, 2-3, 4-5, and 6+. Most recent move was categorized as occurring at 17-18, < 17, and no move age 10+. DGBI diagnoses identified by ICD-9/10 codes included constipation, fecal incontinence (FI), irritable bowel syndrome (IBS), and abdominal pain and were examined as a group and individually (except FI). Logistic regression identified odds of diagnosed DGBIs by most recent move; adjusted models controlled for family demographics, comorbidities and total moves. 

Results: In unadjusted and adjusted analysis, odds of any DGBI diagnosis were increased with female sex, mood disorders, obesity, and lower rank of military parent, and not associated with parental marital status. In unadjusted analysis, any DGBI was not associated with parent sex, ADHD, and 2-3 or 6+ moves, but DGBI was increased with 1 and 4-5 moves. In adjusted analysis, odds of DGBIs increased with ADHD and decreased with having a female military parent and any number of previous moves. In unadjusted analysis, moving at age 17-18 was associated with a 58% increased odds of diagnosed DGBIs; moving age < 17 was not associated with DGBIs. After adjustment, odds of any DGBI were increased 11% in those who last moved age < 17, and 81% in those who last moved at 17-18 as compared to those who had never moved (Table 1). In adjusted analysis, odds of constipation, IBS, and abdominal pain increased by 14%, 33%, and 9% respectively (last move < 17) and 89%, 101%, and 76% respectively (last move 17-18; table 2).Conclusion(s): Recent adolescent moves are associated with increased DGBIs; however, the total number of moves is associated with decreased DGBIs. Results suggest that while moves may increase resilience over time, recent adolescent moves and those during the final years of high school may increase risk. Providers should be conscious of moves when attempting to provide preventative care and treatment of DGBIs.
