Laryngeal mask or similar supraglottic airway devices (LMA/SAD) are recognized in the Neonatal Resuscitation Program as alternatives to tracheal intubation, but also to ineffective face mask ventilation. While trainees’ experience in neonatal intubation has diminished, recent randomized trials have shown the ease of use of LMA in neonatal resuscitation, with immediate effectiveness comparable to endotracheal intubation and post-asphyxia outcomes equivalent to face mask ventilation. Other recent RCTs have demonstrated the effectiveness of Surfactant Administration through LMA/Supraglottic Airways (SALSA approach). Nevertheless, most trainees and neonatal care providers have never used an LMA and are not familiar with the design features of various SADs available for neonatal use.
This workshop combines the experience of 8 neonatologists who have lead research on LMA use in varied neonatal care settings, to briefly review types of neonatal LMA/SAD devices with hands-on demonstrations, discuss the laboratory and clinical evidence on LMA use for neonatal resuscitation and surfactant administration, and demonstrate procedural techniques using video clips and manikin simulations. Small groups of participants will simulate LMA/SAD insertion and surfactant administration in manikins, guided by performance checklists. Case-based large group discussion will illustrate other uses of LMA in neonates, from transport to palliative care. Caveats, obstacles and strategies for implementing LMA use and spreading competencies in the attendees’ local practice settings will be discussed in a large group. Handouts including skills checklists and thematic lists of key references, in addition to a Facebook page, will help attendees in teaching and implementing these skills within their institutional settings.
