Accurate and timely diagnosis of pathogens causing neonatal infection is critical to optimizing patient outcomes. Challenges in diagnosis include the time for traditional culture-based microbiological tests to result, and the persistent issue of continuing treatment for infection even when cultures do not isolate a pathogen. The latter includes a heterogeneous groups of conditions often referred to as “Culture-negative sepsis”. The incidence of this condition is highly variable across centers highlighting the uncertainty around the diagnosis and its use. Both antibiotic overuse and misdiagnosis of the true underlying condition leading to patient decompensation may occur when using this diagnosis. Dr. Benitz will describe the prevalence of culture-negative sepsis diagnosis in neonatal care, the factors driving its use and gaps in knowledge that need to be addresses to decrease or re-define its use. Molecular diagnostics using culture-independent techniques promise to diminish some of the challenges in pathogen detection among neonates. However, despite the many platforms of testing have reached commercialization, routine clinical use suffers from lack of availability and unclear guidance on integration with existing systems. Dr. Lawrence will describe the most promising tests in the field and discuss the challenges in bring them to every-day care.
